ASSESSMENT
The assessment is our way of gaining useful information, and to determine the need for service. Priority is given to those who are at risk of premature admission to a nursing home.
REFERRAL
Once an assessment has been completed we can, with permission, link you with other services.

“Don’t Diabetes Me”
Port Stephens Community Care has been successful in obtaining funding through Port Stephens Council's Aboriginal Fund. “Don’t Diabetes Me” is an Aboriginal specific program. The aim of this program is to provide basic life support skills focusing on nutrition, good health choices and financial discussions that support Diabetes control and reduction to Aboriginal people within the Port Stephens Shire.
The program will run for six weeks with transport and a nutritional meal provided each day. Participants will be provided with food handling and hygiene skills. Selected topics for each day will include Nutrition, Shopping, Food Preparation, Financial Support and Budgeting.
The outcome Port Stephens Community Care would like to achieve is quality of life through the reduction if incidents of Diabetes in the Aboriginal Community through an understanding of the disease and basic life skill methods that are available for avoidance and control of the disease.
The program will show the importance of planning and financial management that allows for control of Diabetes through nutritional awareness that leads to good food shopping and cooking that reduces risk of diabetes, “Quality of Life.”
