Becoming a Client

The HACC Client Information Assessment and Referral Record (CIARR) or ONI will be used to assess clients

Wherever possible, multiple assessments should be avoided through negotiation and coordination with other agencies involved in providing service to the client.  Where possible, and with the client’s permission, assessment information obtained by other agencies shall be used.

Following initial contact with the organisation, the client will be contacted within two working days to arrange an assessment.  Where the client is in need of immediate service, the necessary details to commence service will be taken and an assessment arranged as soon as possible.  Clients will be given choices within service options.

The assessment shall take place within seven working days of the initial contact and shall be completed in the clients own home.  Clients referred by Community Care Packages will not require an assessment as service is provided at full cost recovery to Community Aged Care Clients.

The assessment is between the organisation, the client, and with their permission, his/her legal guardian or advocate.  Use shall be made of interpreter service when required.
